FILE A CLAIM

Pursuant to the Court's Order Granting Plaintiffs' Unopposed Motion for Preliminary Approval of Class Action Settlement, for Issuance of Notice to the Settlement Class, and for Scheduling of Fairness Hearing, dated April 14, 2025, Settlement Class Members who wish to participate in the Settlement shall complete and submit the Proof of Claim and Release form ("Claim Form") in accordance with the instructions contained therein. You may submit your Claim Form using any one of the following methods. Each method is described below, with links to the requirements for each submission. Your submitted Claim Form will include a certification under penalty of perjury that your Claim Form is accurate and complete.  

Whichever submission method you choose, you are required to provide documentation to support your claimed transactions. Please remember to provide supporting documentation of all transactions listed on your Claim Form. In the event that you submit your claim online, you must retain confirmation of such filing.

If you fail to execute and submit a timely, properly completed and addressed Claim Form, your claim may be rejected and you may be precluded from any recovery from the Net Settlement Fund.

SUBMIT YOUR CLAIM FORM ONLINE

To submit a Proof of Claim online, you must complete the online Proof of Claim found here.  It is estimated that this process will take approximately thirty (30) minutes, though it may take longer if you have a large number of transactions.  Once you begin the online Proof of Claim submission process, you will not be able to save and return to your session.  Your session will time out after one (1) hour and you will not be able to return to the claims filing process.  Please have all necessary information available and be prepared to complete the process at one time. Required documentation must be uploaded in PDF format only.  In order to be timely, your Proof of Claim must be submitted online by August 4, 2025.  

SUBMIT YOUR CLAIM FORM BY MAIL

To submit a claim by mail, complete and sign the Claim Form in its entirety, with all required supporting documentation and mail it, to be postmarked no later than August 4, 2025, to the Claims Administrator at:
